FY 2020 Rural Health Care Coordination Program

Federal Office of Rural Health Policy

The Federal Office of Rural Health Policy is pleased to announce the FY 2020 Rural Health Care Coordination Program, which is now open on grants.gov (announcement number: HRSA-20-030) and is accepting applications through March 12, 2020.  A total of $7.5 million in funding is anticipated to be available for up to 10 awards to support rural health consortiums to improve patient care and disease outcomes using care coordination strategies. Applicants are required to coordinate the health care delivery services in rural communities through the following four focus areas of collaboration, leadership and workforce, improved outcomes, and sustainability. Additional program objectives include: enhanced integrated systems to collaborate and to share data among member organizations; effective care coordination workforce to meet needs within the rural communities; improved access, delivery, and quality of services and overall patients’ health outcomes; and increased program financial sustainability. Eligibility for the program requires that applicants must be either public or nonprofit private rural entities who represent a consortium of at least three separate health care entities and must not previously have received a grant under this program for the same or similar project.  Eligible applicants may request up to $250,000 annually for the program’s 3-year period of performance September 1, 2020-August 31, 2023.

Iowa Walking College

The Healthiest State Initiative and Active Living Iowa


The Iowa Walking College is an interactive educational program created to increase the number of walking advocates in local communities across the state. The program was launched as a pilot program in 2017 and built around the National Walking College curriculum created by America Walks.

Twenty fellowships will be awarded to Iowans working as community change agents. The fellows may be working alone, in organizations, or in professions such as public health, development, planning, transportation, economic development or education. The Iowa Walking College is a six-month program, where fellows will explore ways to improve walkability around the state through online and face-to-face interaction.

DentaQuest

The DentaQuest Foundation makes grants to a variety of organizations that are engaged in improving oral health. The Foundation does not make grants directly to individuals and does not support capital expenses, operating expenses, direct service, mobile vans or payments for procedures. Applicants must have 501(c)3 non profit status.

HealthConnect

Mid-Iowa Health Foundation


Mid-Iowa Health Foundation has launched HealthConnect, a grant program to fund local, innovative partnerships and programs that address social determinants of children’s health. The funding priority is part of the foundation’s five-year strategic plan that aims to improve the health and development of vulnerable children living in Greater Des Moines. 

HealthConnect encourages the formation of unlikely partnerships focused on exploring non-health solutions to the fundamental causes of poor health for children and youth: unsafe housing and neighborhoods, inadequate health and education services and continued exposure to toxic levels of stress, among other challenges.
